WHY: In the mid-1980s, one out of 50 babies in New Haven died within their first year of birth, an Infant Mortality Rate (IMR) of more than 20 deaths per 1000 live births. Three decades later, thanks to the New Haven Healthy Start program, the health of thousands of newborns has been improved and the IMR has fallen to seven deaths per 1000 live births or one out of 150 babies. This powerful story is one of hope, determination and accomplishment in the field of maternal and child health.  It shows how in a single generation, a motivated community went from having one of  the nation’s highest infant mortality rates to significantly reducing mortality rates by implementing a multi-faceted system that now serves as a care coordination model for other states.
