The common grant application and report forms were developed by the Connecticut Council for Philanthropy and an advisory committee of foundation and nonprofit professionals working to simplify the grantseeking and reporting processes. They are for use solely by Connecticut nonprofit organizations.


Common Grant Application Form

The Common Grant Application Form was updated to a fillable form Word document in 2011. The application must be sent to the funder from which the organization is requesting funds.


Common Report Form
The Common Report Form was updated to a fillable form Word document in 2011. The report must be sent to the funder from which the organization received funds.
